“If only someone had told us”: Transitioning to end-of-life care

In this very powerful Pecha Kucha presentation, AMS Phoenix Fellow, Dr. James Downar tells us about the importance of end-of-life care transitions. He reminds us that patients want to transition to an end-of-life care approach at exactly the right time. They want to prepare for their deaths, and if they receive less aggressive care, they are often able to have a better quality of life before they die. 

Hear about the support tool James built using his AMS funding. It identifies patients who might be nearing the end of their lives and “nudges” their care teams to consider talking with these patients about changing their care approach. It’s a powerful example of how a small technological intervention can have a major impact.

This talk will most certainly make you consider the decisions you might make if you knew the end was near.
