Many people diagnosed with breast cancer face difficulties after finishing hospital-based treatment as they transition into primary care. Whether they continue on long-term hormone therapy or have completed treatment after surgery and radiation, many feel unprepared, unsupported, and lost. Ongoing side effects, like fatigue, joint pain, or emotional distress, can affect quality of life and make it hard to stick with treatment plans.
Quynh and her team aim to support people during this critical time by co-creating a compassionate, nurse-led, technology-facilitated solution that helps manage side effects in a way that feels personal and relevant. They are co-designing with people previously diagnosed with breast cancer, caregivers, and healthcare providers a digital program that fits real-life needs and strengthens relationships. Equity is central to this work. Their goal is to make sure every person affected by breast cancer has access to the right support, at the right time, in the right way.
