Empowering patients and caregivers in safer care transitions from hospital to home through the scaling of patient

Transitioning from hospital to home is challenging for patients, who manage medications, routines, and follow-up care. Discharge summaries are written for family doctors and are often difficult for patients to understand. Patient-oriented discharge summaries (PODS) provide clear, patient-friendly instructions but are not widely used because clinicians lack time to prepare them. Jeremy’s project uses generative artificial intelligence to create PODS from medical records for clinician review, helping scale PODS, empower patients, and improve care experiences.
