Advancing Compassion by design: An AI-driven frailty score to advance equitable care for people experiencing homelessness

People experiencing homelessness often have complex health needs that may not be recognized by existing healthcare tools. Stephanie’s project will develop and test an AI-tool to identify people who may be at risk of poor health outcomes. The tool will use routinely collected health data and will be developed in partnership with people who have experienced homelessness, frontline service providers, and researchers. The AI tool will consider factors such as chronic health conditions, healthcare use, mental health, and substance use. Her project will also identify important challenges and stressors that are not captured in routinely collected health data, helping to identify gaps in current knowledge and data. By focusing on both accuracy and ease of understanding, the tool aims to help healthcare and shelter providers identify people who may need support earlier. Stephanie’s equity-focused project will combine data and lived experience to improve care and better support people experiencing homelessness.
